DEFINITION

Chronic heart failure is a clinical syndrome of breathlessness, fatigue and fluid retention caused by structural or functional cardiac impairment.

DIAGNOSIS

Measure NT-proBNP in all people with suspected heart failure. Refer for specialist assessment and echocardiography within two weeks if above 2,000 ng/litre.

Offer an ACE inhibitor and a beta-blocker licensed for heart failure as first-line treatment.
MANAGEMENT

If symptoms persist, add an aldosterone antagonist and monitor potassium and renal function. Refer to a specialist heart failure multidisciplinary team.
